Member changes their PCP

When a member seeks care from you but isn't assigned to you as their PCP yet, encourage the member to submit a PCP change request. You may submit a PCP change form on the member's behalf once the member completes the form. This is especially common and important for Medicaid and Healthy Michigan Plan members.

  1. The member completes the Primary Care Provider Change Form
  2. You (or the member) fax the completed form to us

Phone The member calls Customer Service at the number on the back of their member ID card The day the request is made
